Living Will

 A document that defines the circumstances under which health care professionals should withhold or remove artificial life support, or refrain from using heroic measures, when the patient is unable to give informed consent to the termination of life support  due to incapacity. A common circumstance is a terminal illness with no expectation of recovery.  The directives of the living will are usually executed by the person's appointed health care agent or proxy.

A living will is also known as a natural death declaration.
